1. Agreement
By accessing hillsideprimarycare.com, affiliated clinic websites, patient portals, or opting into SMS messages from Hillside, you consent to these Terms & Conditions. Declining means you should not use the site or enroll in SMS services.
Medical services operate under a separate treatment relationship. These Terms do not supersede privacy notices, treatment consent forms, or other clinical documentation.
2. SMS Terms
Hillside Primary Care, PLLC d/b/a Hillside Medical Group operates this SMS program.
SMS program terms
Program / message types. Following opt-in, we may transmit appointment reminders, schedule modifications, telehealth visit notifications with access details, patient-portal or intake alerts, and billing reminders. Messages exclude diagnostic or test findings. Third-party marketing is not included.
Who messages come from. Hillside Primary Care, PLLC d/b/a Hillside Medical Group sends messages. Related practice names appearing in materials include Psychiatry of SA, Women's Wellness of SA, and Podiatry of SA. SMS campaigns display as Hillside Medical Group.
Opt-in. You opt in only if you mark an empty SMS checkbox on web or paper intake/scheduling forms, or provide verbal authorization using our staff script, followed by a confirmation text. Consent is voluntary and not required for receiving care, scheduling, or treatment.
Frequency. Message volume fluctuates based on scheduled appointments and account activity.
Rates. Standard message and data charges may apply.
Help. Reply HELP or contact 210-742-6555 or office@hillsidemedicalgroup.com.
Opt-out. Reply STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT. You'll get confirmation and no additional program communications. Request removal at the front desk as well.
Carriers. Wireless carriers bear no responsibility for message delays or failures.
SMS data. Personal information and SMS opt-in data are not sold or distributed to third parties for promotional purposes.

3. Website use
Website content provides general information regarding our clinics and services. This material is informational only and does not constitute medical guidance or establish a clinician-patient relationship. Do not rely on the website during emergencies. Prohibited activities include site misuse, unauthorized system access, and submitting another individual's information without authorization.
4. Privacy
Our Notice of Privacy Practices describes how we handle and disclose protected health information. You can read it in our registration forms (Privacy Notice).
Mobile numbers provided for SMS enrollment are used exclusively to deliver consented messages and process STOP and HELP directives.
5. Appointments and financial policy
Scheduling, cancellation, no-show, insurance, and payment guidelines appear in patient intake materials you sign at the clinic. These clinical and financial documents take precedence if they conflict with this webpage, except SMS authorization is governed by Section 2's SMS Terms.
6. Changes
We may revise these Terms. The latest version appears at this URL with an updated effective date. Continuing to use the website or the SMS program following an update indicates acceptance. SMS enrollment can be terminated anytime by replying STOP.
