This Privacy Policy explains how we collect and use information through hillsideprimarycare.com and our patient text message (SMS) program. How we use and disclose protected health information is described in our Notice of Privacy Practices, which controls for all medical information.
Information we collect
- Contact details you give us, such as name, mobile number, email address and date of birth, when you register, book an appointment, complete a form or opt in to text messages.
- Your SMS consent record: the date, the staff member who recorded your verbal consent, and the mobile number enrolled.
- Basic website usage data, such as pages visited and browser type.
How we use it
- To schedule and remind you of appointments, send check-in and telehealth instructions, patient-portal and intake-form notices, and billing reminders you agreed to receive.
- To respond to your questions and process HELP and STOP requests.
- To operate and improve our website and services.
Text messaging (SMS)
Mobile numbers collected for our SMS program are used only to send the messages you consented to. Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to opt out at any time. Consent is not a condition of receiving care.
We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes. SMS opt-in data and consent are not shared with any third party, except the service providers who deliver our messages under a business associate agreement.
How we protect it
We use administrative, technical and physical safeguards consistent with HIPAA. Vendors who handle patient information for us sign business associate agreements.
Your choices
You may opt out of texts by replying STOP, ask the front desk to remove your number, or contact us to update your information.
